WHAT IS YOUR COVID-19/CORONAVIRUS POLICY?
Your health and safety are my number one priority and I have put in place the following steps to ensure your safety:
You complete our Coronavirus screening form prior to each visit
I come to you! The convenience of in-home or virtual services decreases the risk of exposure.
I complete a second Coronavirus screening form when I am with you and check your temperature
I will be wearing masks, face shield, and gloves during all sessions.
I will be sanitizing all equipment before and after each session.
I urge all clients to take proper precautions as well and wear a mask or face covering.
DO YOU PROVIDE TELE-HEALTH SERVICES?
Yes! I can provide all of our services using video calls!
WHAT IS IN-HOME PHYSICAL THERAPY AND DO I NEED A PRESCRIPTION OR REFERRAL FROM MY PHYSICIAN?
In-home physical therapy means that you will receive exceptional outpatient physical therapy services from the comfort of your home. There is no need to travel, I come to you. This convenience allows you more flexibility when scheduling and provides a safe way for you to get the care you need.
NO! You do not need a referral from your physician to start physical therapy services although it is always best to keep your physicians in the loop concerning your health and any care you are receiving.

WHAT IS YOUR NEW PATIENT PROCEDURE?
First I complete a consultation session via phone or video call where we discuss your current issue, general history, and goals to make sure I can serve your needs optimally
Following our call, you will receive general intake forms and access to the patient portal.
Lastly, I schedule your initial evaluation and start your journey toward optimizing your health!
DO YOU PROVIDE SERVICES TO MEDICARE BENEFICIARIES?
YES! I am a Medicare Part B provider! If you have traditional Medicare you are covered!
If you have a Medicare Advantage Plan- most plans have out of network benefits!
I will verify your insurance coverage so that you can submit an invoice to your insurance provider for reimbursement.
DO YOU ACCEPT INSURANCE?
If you have it, you can use your out of network insurance benefits to reimburse you for services I provide!
I will verify your insurance coverage for you so that you can have as much cost transparency as possible!
You can also use your Health Savings Account (HSA)/Flexible Spending Account!
WHAT FORMS OF PAYMENT DO YOU ACCEPT?
I accept Credit Cards (Visa, Mastercard, Discover, American Express),Debit Cards, and HSA/FSA. Make sure you speak with your HSA/FSA provider about eligible expenses prior to use.
DO I NEED ANY EQUIPMENT?
No! I provide all of the equipment you will need to start your health and wellness or rehabilitation journey
